The personal data I record about you including data relating to your name, address, date of birth, wider contact details and data relating to your ‘health' will be stored in paper form.
This data will only be used for the purpose for which it is collected, in confidence and not supplied or shared with any other third parties.
I do not carry out any form of direct marketing activities and will only respond to emails you send me.
This data will be kept for at least 7 years following the last occasion on which treatment was given. In the case of treatment to minors for 7 years after they reach the age of majority (18).
